About the role
Key responsibilities & impact- Implement clinical and individual programs, ISPs, daily schedules, and recommended plans
- Provide support, instruction, and skill building to persons served in day programs
- Assist with case management and interdisciplinary team reviews
- Assist in developing ISPs and maintain contact with families, funding sources, and staff
- Implement clinical and daily plans and recommend program improvements
- Maintain daily documentation, data collection, and progress notes
- Work individually and in groups to develop skills and keep members engaged in meaningful activities
- Communicate with management regarding treatment and programs
- Participate in case management and other teams
- Support members in crisis using Positive Behavior Support and Safety Care
- Assist with staff training
- Maintain program-area upkeep and monitor, clean, and store supplies and equipment
- Advocate for persons served and support community integration
- Perform other related duties as required
Requirements
What you’ll need- One year of relevant work experience/training
- High school diploma or equivalent required
- Valid US driver's license presented at employment
- Adequate insurance and safe driving record if using a personal vehicle
- At least 21 years of age to drive
- Valid US driver's license maintained for at least six months
- Must pass a driver's screening background check
- CPR certification required within two weeks of hire
- First Aid certification required within two weeks of hire
- Safety Care certification required within 90 days of hire
- NET certification required
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to assist clients with daily living tasks
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Required close, distance, color, peripheral, and depth vision, plus ability to adjust focus
- Knowledge of advocacy, applied behavioral analysis, communication styles, empowerment, assessment, participatory planning, physical and independent living management, and crisis intervention
- Persuasive, collaborative, and presentation skills
- Ability to work independently and as part of an interdisciplinary team
- Ability to provide a positive role model
