About the role
Key responsibilities & impact- Implement clinical and individual programs, including the daily DHSP schedule and recommended plans
- Provide instruction and skill building in self-help, sensorimotor, communication, social development, independent living, behavior development, affective development, and wellness development
- Assist with case management responsibilities and provide input for interdisciplinary team reviews
- Assist in developing DHSPs for persons served and maintain contact with families, funding sources, and staff
- Implement clinical and daily plans and recommend or implement program changes for continuous improvement
- Maintain daily documentation, data collection, and progress notes according to guidelines
- Work directly with persons served individually and in groups to develop skills
- Keep members engaged in meaningful activities and offer choices where possible
- Communicate with management regarding treatment and programs
- Participate in case management and other interdisciplinary teams
- Support members in crisis management using Positive Behavior Support (PBS) and Safety Care
- Assist with staff training as needed
- Maintain program-area upkeep and monitor, clean, and properly store supplies and equipment
- Advocate for persons served and support community integration through community and volunteer opportunities, reverse integration, and friendships
- Perform other related duties as required
Requirements
What you’ll need- One year of relevant work experience/training
- High school diploma or equivalent required
- Valid driver's license presented at employment
- At least 21 years of age
- Valid US driver's license maintained for at least six months
- Adequate insurance and a safe driving record if using a personal vehicle
- Pass a driver's screening background check
- Ability to drive a Vinfen van or personal vehicle
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to assist clients with daily living tasks
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Ability to operate a computer, calculator, copier, and printer
- Required vision abilities including close, distance, color, peripheral, and depth vision, depth perception, and focus adjustment
- CPR within 2 weeks of hire
- First Aid within 2 weeks of hire
- New Employee Training (NET)
- Safety Care within 90 days of hire
- Knowledge of advocacy techniques
- Knowledge of applied behavioral analysis with developmental disabilities populations
- Knowledge of different communication styles
- Knowledge of empowerment and self-advocacy techniques
