About the role

Key responsibilities & impact

Provide instruction and skill building in self-help, sensorimotor, communication, social development, independent living, behavior development, affective development, and wellness development
Assist with case management responsibilities
Provide input for interdisciplinary team reviews
Assist in developing DHSPs for persons served
Maintain contact with family members, funding sources, and staff members
Implement clinical and daily plans and recommend program improvements
Maintain daily documentation and progress notes
Work with persons served individually and in groups to develop skills
Keep members engaged in meaningful activities and offer choices
Communicate with management regarding treatment and programs
Participate in case management and other teams
Support members in crisis management using Positive Behavior Support and Safety Care
Assist with staff training as needed
Maintain program area, supplies, and equipment
Advocate for persons served and support community integration
Perform other related duties as required

Requirements

What you’ll need

One year of relevant work experience/training
High school diploma or equivalent required
Valid driver's license presented at employment
Adequate insurance and safe driving record if using a personal vehicle
At least 21 years of age
Valid US driver's license maintained for at least six months
Ability to pass a driver's screening background check
Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and navigate stairs and narrow spaces
Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
Ability to assist clients with activities of daily living
Ability to operate a computer, calculator, copier, and printer
Ability to speak, hear, and communicate with clients, staff, and external representatives
Required vision abilities including close, distance, color, peripheral, and depth perception, plus focus adjustment
CPR within 2 weeks of hire
First Aid within 2 weeks of hire
New Employee Training (NET)
Safety Care within 90 days of hire
Knowledge of advocacy, applied behavioral analysis, communication styles, empowerment, assessment, participatory planning, physical and independent living management, and crisis intervention
Ability to work independently and as part of an interdisciplinary team
Ability to provide a positive role model
