Key responsibilities & impact
  • Provide instruction and skill building in self-help, sensorimotor, communication, social development, independent living, behavior development, affective development, and wellness development
  • Assist with case management responsibilities
  • Provide input for interdisciplinary team reviews
  • Assist in developing DHSPs for persons served
  • Maintain contact with family members, funding sources, and staff members
  • Implement clinical and daily plans and recommend program improvements
  • Maintain daily documentation and progress notes
  • Work with persons served individually and in groups to develop skills
  • Keep members engaged in meaningful activities and offer choices
  • Communicate with management regarding treatment and programs
  • Participate in case management and other teams
  • Support members in crisis management using Positive Behavior Support and Safety Care
  • Assist with staff training as needed
  • Maintain program area, supplies, and equipment
  • Advocate for persons served and support community integration
  • Perform other related duties as required

Requirements

What you’ll need
  • One year of relevant work experience/training
  • High school diploma or equivalent required
  • Valid driver's license presented at employment
  • Adequate insurance and safe driving record if using a personal vehicle
  • At least 21 years of age
  • Valid US driver's license maintained for at least six months
  • Ability to pass a driver's screening background check
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and navigate stairs and narrow spaces
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to assist clients with activities of daily living
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • Required vision abilities including close, distance, color, peripheral, and depth perception, plus focus adjustment
  • CPR within 2 weeks of hire
  • First Aid within 2 weeks of hire
  • New Employee Training (NET)
  • Safety Care within 90 days of hire
  • Knowledge of advocacy, applied behavioral analysis, communication styles, empowerment, assessment, participatory planning, physical and independent living management, and crisis intervention
  • Ability to work independently and as part of an interdisciplinary team
  • Ability to provide a positive role model
