About the role
Key responsibilities & impact- Implement clinical and individual programs, including the daily DHSP schedule and recommended plans
- Provide instruction and skill building in self-help, sensorimotor, communication, social, independent living, behavior, affective, and wellness development
- Assist with case management responsibilities and provide input for interdisciplinary team reviews
- Assist in developing DHSPs for persons served and maintain contact with families, funding sources, and staff
- Implement clinical and daily plans and recommend or implement program improvements
- Maintain daily documentation, data collection, and progress notes
- Work individually and in groups to develop skills and engage persons served in meaningful activities
- Support crisis management using Positive Behavior Support (PBS) and Safety Care
- Assist with staff training as needed
- Maintain program area, supplies, and equipment
- Advocate for persons served and support community integration, volunteer opportunities, reverse integration, and friendships
- Participate in case management and other interdisciplinary teams
- Communicate with management regarding treatment and programs
- Perform other related duties as required
Requirements
What you’ll need- One year of relevant work experience/training
- High school diploma or equivalent required
- Valid driver's license presented at employment
- At least 21 years of age
- Maintained a valid US driver's license for at least six months
- Adequate insurance if using a personal vehicle
- Safe driving record and ability to pass an annual driver's screening background check
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to assist clients with daily living tasks
- Ability to operate a computer, calculator, copier, and printer
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Required vision abilities including close, distance, color, peripheral, and depth perception, plus ability to adjust focus
- Knowledge of advocacy techniques, applied behavioral analysis with developmental disabilities, communication styles, empowerment and self-advocacy, assessment practices, participatory planning, physical personal management, independent living management, and crisis intervention
