About the role
Key responsibilities & impact- Develop and implement person-centered treatment care plans
- Provide daily nursing care and assistance with activities of daily living, including showering, dressing, diabetes care, wound care, GT/JT care, trach care, and nutritional support
- Monitor clients for changes in status and perform individualized assessments
- Administer medications and treatments as ordered by the primary care provider
- Provide transportation and accompany individuals to medical, dental, and other appointments
- Ensure nutritional needs, medical supplies, equipment, and treatments are managed safely and timely
- Utilize behavioral strategies and follow prescribed clinical plans
- Supervise CNAs during the designated shift and provide nursing education and training
- Communicate with program leadership, providers, families, guardians, agencies, and Vinfen employees
- Document nursing care, medication administration, treatment plans, medical records, shift notes, and incident reports
- Maintain a safe, clean, welcoming, and regulation-compliant environment
- Support community outings and events and respect individual preferences
- Advocate for individuals served and collaborate with Vinfen departments and community providers
- Participate in treatment-plan, ISP, and other planning meetings
- Maintain effective relationships with individuals served, families, guardians, and community contacts
- Comply with accreditation, licensing, fire, safety, and Vinfen policy requirements
Requirements
What you’ll need- Massachusetts LPN license required
- High school diploma or equivalent required; Associate’s degree preferred
- CPR certification required within two weeks of hire
- Safety Care certification required within 90 days of hire
- May be asked to obtain CBIS certification
- One to two years of supervised practical experience, including internships, helpful but not required
- Experience with people with brain injury preferred
- Experience with developmental and/or psychiatric disabilities, medical illnesses, and co-occurring disorders preferred
- Valid driver’s license required at employment
- Must be at least 21 years of age
- Must have maintained a valid US driver’s license for at least six months
- Must pass a driver’s screening background check
- Adequate insurance and a safe driving record required when using a personal vehicle
- Ability to supervise and mentor CNAs
- Knowledge of LPN practice standards, intervention techniques, evidence-based practices, assessment practices, documentation standards, and community health and mental health services
- Ability to make independent judgments and decisions and work professionally and confidentially
- Ability to use personal computer applications and office equipment
- Ability to perform physically active duties, including standing, walking, bending, kneeling, lifting at least 25 pounds, assisting with activities of daily living, and maneuvering through narrow spaces
- Ability to communicate with clients, staff, and external representatives
