About the role

Key responsibilities & impact

Assist in developing, implementing, and documenting individual treatment support plans
Perform daily tasks with persons served, including rehabilitative and behavioral interventions, appointment scheduling, progress monitoring, transportation, and related support
Provide role modeling and support, helping persons served integrate into their communities
Assess physical and emotional status and report changes to a supervisor
Ensure program site safety and maintenance
Participate in daily living and housekeeping activities, including cooking, meal planning, grocery shopping, lawn mowing, snow shoveling, and garbage removal
Comply with corporate and divisional policies, safe work practices, workers’ compensation, universal precautions, and exposure control requirements
Store, administer, document, and dispose of medication according to MAP requirements, funding-source regulations, and company policy
Complete communication logs, community membership logs, chore schedules, timesheets, and application forms
Perform crisis intervention as needed
Provide current contact information to the Relief Pool Coordinator or Human Resources staff
Perform other related duties as required

Requirements

What you’ll need

One to three months of experience in a human services setting
Experience may be substituted for academic training in some cases
High school diploma or equivalent required
Valid US driver’s license must be presented at employment if driving
At least 21 years of age if driving
Valid US driver’s license maintained for at least six months if driving
Ability to pass a driver screening background check if driving
Safe driving record if driving
Adequate insurance if using a personal vehicle
Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
Ability to operate a computer, calculator, copier, and printer
Ability to assist clients with daily living tasks
Ability to speak, hear, and communicate with clients, staff, and external representatives
Required vision abilities including close, distance, color, peripheral, and depth perception, plus ability to adjust focus
First Aid required within two weeks of hire
CPR required within two weeks of hire
MAP required within 150 days of hire
Safety Care required within 90 days of hire
Knowledge of advocacy, communication, empowerment, self-advocacy, physical personal management, household management, and crisis intervention techniques
Sensitivity to cultural, religious, racial, disability, and gender issues
Teaching skills
Ability to document interventions
Ability to work independently and as part of an interdisciplinary team
Ability to provide role model behaviors
