About the role
Key responsibilities & impact- Assist in developing, implementing, and documenting individual treatment support plans
- Perform daily habilitative and rehabilitative tasks with persons served, including behavioral interventions, appointment scheduling, progress monitoring, and transportation
- Provide role-model support and help persons served integrate into their communities
- Assess physical and emotional status and report changes to a supervisor
- Maintain program-site safety and participate in daily living and cleaning tasks, including cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal
- Comply with corporate and divisional policies, safe work practices, workers’ compensation, universal precautions, and exposure-control procedures
- Store, administer, document, and dispose of medication in accordance with MAP requirements, funding-source regulations, and company policy
- Complete communication logs, community membership logs, chore schedules, timesheets, and application forms
- Perform crisis intervention as needed
- Provide current contact information to the Relief Pool Coordinator or Human Resources
- Perform other related duties as required
Requirements
What you’ll need- One to three months of experience in a human services setting
- Experience may be substituted for academic training in some cases
- High school diploma or equivalent required
- Valid US driver’s license must be presented at employment if driving
- At least 21 years of age if driving
- Valid US driver’s license maintained for at least six months if driving
- Ability to pass a driver screening background check if driving
- Safe driving record if driving
- Adequate insurance if using a personal vehicle
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to assist clients with daily living tasks
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Required vision abilities include close, distance, color, peripheral, and depth perception, plus ability to adjust focus
- First Aid required within two weeks of hire
- CPR required within two weeks of hire
- MAP required within 150 days of hire
- Safety Care required within 90 days of hire
- Knowledge and use of advocacy, communication, empowerment, self-advocacy, physical personal management, household management, and crisis intervention techniques
- Teaching skills
- Ability to document interventions
- Ability to work independently and as part of an interdisciplinary team
- Ability to provide role-model behaviors
- Sensitivity to cultural, religious, racial, disability, and gender issues
