About the role

Key responsibilities & impact

Assist in developing, implementing, and documenting individual treatment support plans
Perform daily tasks with persons served, including rehabilitative and behavioral interventions, appointment scheduling, progress monitoring, transportation, and related support
Serve as a role model and support persons served in becoming integrated into their communities
Assess physical and emotional status and report changes to a supervisor
Ensure program site safety and maintenance
Oversee and participate in daily living and cleaning activities, including cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal
Comply with corporate and divisional policies, safe work practices, workers’ compensation, universal precautions, and exposure control procedures
Store, administer, document, and dispose of medication according to MAP requirements, funding-source regulations, and company policy
Complete communication logs, community membership logs, chore schedules, timesheets, and application forms
Perform crisis intervention as needed
Maintain current contact information with the Relief Pool Coordinator or Human Resources
Perform other related duties as required

Requirements

What you’ll need

One to three months of experience in a human services setting
Experience may substitute for academic training in some cases
High school diploma or equivalent required
Valid driver’s license presented at employment if driving
At least 21 years of age if driving
Valid US driver’s license maintained for at least six months if driving
Pass a driver screening background check if driving
Maintain a safe driving record if driving
Adequate insurance if using a personal vehicle
Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces
Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
Ability to operate a computer, calculator, copier, and printer
Ability to assist clients with daily living tasks
Ability to speak, hear, and communicate with clients, staff, and external representatives
Required vision abilities including close, distance, color, peripheral, and depth perception, plus ability to adjust focus
First Aid certification required within two weeks of hire
CPR certification required within two weeks of hire
MAP certification required within 150 days of hire
Safety Care certification required within 90 days of hire
Knowledge and use of advocacy techniques
Knowledge and use of different communication styles
Knowledge of empowerment and self-advocacy techniques
Knowledge of physical personal management skills
Knowledge of household management skills
Knowledge of crisis intervention techniques
Sensitivity to cultural, religious, racial, disability, and gender issues
Teaching skills
Ability to document interventions
Ability to work independently and as part of an interdisciplinary team
Ability to provide role model behaviors
