About the role

Key responsibilities & impact
  • Provides clinical oversight and consultation to the agency’s centralized intake and engagement team.
  • Reviews and triages referrals to determine appropriateness for agency programs and level of care.
  • Supports timely connection to internal services including care coordination, behavioral health, housing supports, HRSN services, and community-based programs.
  • Coordinates referrals to external providers and community partners when individuals’ needs fall outside the agency’s scope of services.
  • Assists staff in identifying urgent clinical, behavioral health, substance use, housing instability, or safety concerns requiring escalation or immediate intervention.
  • Develops and maintains collaborative relationships with hospitals, health plans, behavioral health providers, shelters, primary care practices, and community agencies.
  • Monitors referral workflows, intake timelines, and engagement outcomes to improve access and continuity of care.
  • Participates in interdisciplinary case reviews and case conferencing.
  • Provides clinical supervision, coaching, and support to Community Health Workers and other paraprofessional staff.
  • Supports staff in engagement strategies, motivational interviewing, crisis intervention, care planning, boundary setting, and trauma-informed approaches.
  • Ensures services are delivered in accordance with MassHealth, agency, contractual, and regulatory requirements.
  • Reviews documentation for quality, compliance, and clinical appropriateness.

Requirements

What you’ll need
  • Minimum of 3 years of experience working with individuals with behavioral health, substance use, medical, and/or complex social needs.
  • Experience supervising or mentoring staff in community-based or behavioral health settings.
  • Strong clinical assessment, crisis intervention, and care coordination skills.
  • Excellent communication, organizational, and interdisciplinary collaboration skills.
  • Ability to work independently and manage competing priorities in a fast-paced environment.
  • Valid driver’s license and reliable transportation required.
  • Master’s degree in social work, Counseling, Psychology, Human Services, or related behavioral health field required.
