About the role

Key responsibilities & impact
  • Ensure the overall integrity, safety, rationality, and effectiveness of the drug use control process for assigned patients
  • Monitor, screen, and verify drug-related orders for proper dosage, route, dosage form, and administration schedule
  • Prepare and dispense dosage forms and resolve drug therapy problems and problematic drug orders
  • Monitor medication effects and recommend or initiate adjustments to achieve desired outcomes
  • Provide comprehensive clinical pharmacy services for acute care pediatric patients, including medication selection, dosing, and therapeutic monitoring
  • Participate in daily interdisciplinary rounds and evidence-based treatment planning
  • Manage and optimize high-risk medication therapies
  • Support transitions of care through medication reconciliation, discharge counseling, and continuity planning
  • Educate medical staff, trainees, and families on pediatric pharmacotherapy and medication safety
  • Lead or support quality improvement initiatives, research projects, and clinical guideline development
  • Supervise pharmacy interns, certified pharmacy technicians, and pharmacy residents as applicable
  • Interact with healthcare practitioners in a multidisciplinary environment

Requirements

What you’ll need
  • Current licensure with the Virginia State Board of Pharmacy required or obtained within the first 90 days of employment
  • Licensed as a Pharmacist with the Virginia Board of Pharmacy for continued employment
  • Bachelor of Science in Pharmacy from an accredited academic program
  • Strong clinical knowledge and experience in pediatric acute care or pediatric hospital medicine
  • Excellent communication, collaboration, and critical thinking skills
  • Must be available to work evenings, nights and weekends
  • Ability to lift 20–50 lbs.
  • PGY1 or PGY2 ASHP-accredited residency preferred
  • Doctor of Pharmacy (Pharm.D.) from an accredited academic program preferred in the licensure section
  • Completion of a PGY-2 residency in Pediatric Pharmacy or Pediatric Critical Care preferred
  • Board certification (BCPPS or equivalent) highly desirable
