About the role
Key responsibilities & impact- Provide direct patient care to patients and families
- Assist in coordinating team-delivered services
- Complete psychosocial assessments
- Develop treatment plans congruent with the overall healthcare plan
- Provide psychosocial interventions
- Educate patients, family members, staff, and social work students
- Facilitate compliance with treatment
- Work with the community to provide needed services
- Coordinate discharge planning activities
- Initiate patient/family interventions based on assessed service needs
- Initiate community and facility referrals
- Consult with facility staff regarding psychosocial patient/family needs
- Report suspected abuse, neglect, and exploitation cases to Social Services
- Develop intervention plans with patients, families, and significant others
- Initiate education activities for facility staff
- Contribute to quality improvement through self-analysis of services and needs
- Participate in service area meetings, training, and patient care conferences
- May supervise social work students and students from other disciplines depending on assignment
Requirements
What you’ll need- Licensed Clinical Social Worker in the State of Virginia preferred
- Minimum of three (3) years of clinical social work experience
- At least one (1) year of experience in a specialty practice area
- Master of Social Work Degree from a Council on Social Work Education accredited program
- Clinical social work experience in an acute care setting preferred
- May be required to work alternate schedules, weekends and/or holidays, and take call
- May be required to work alternate unit assignments, including at other clinical locations
- Ability to lift less than 20 lbs.
- Prolonged standing and sitting; frequent bending, walking, and overhead/extensive/repetitive reaching
- Strong recall, reasoning, problem solving, hearing, clear speech, legible writing, reading, and logical thinking
- Ability to work in a fast-paced, noisy environment, handle multiple priorities, manage frequent and intense customer interactions, and adapt to frequent change
