About the role

Key responsibilities & impact
  • Triage new patient appointments to appropriate providers and ensure timely scheduling
  • Review outside medical records, test results, and documentation for initial visits
  • Assess barriers to care and facilitate referrals to support services and community organizations
  • Educate patients and families about diagnoses, treatment plans, treatment options, side effects, survivorship, wellness, and prevention
  • Support patients throughout the care continuum, including active treatment, survivorship, chronic cancer management, and end-of-life care
  • Coordinate care with multidisciplinary teams, physicians, caregivers, staff, and referring providers
  • Facilitate scheduling of appointments, diagnostic testing, procedures, and clinical trial referrals
  • Track patient progress in EPIC and ensure key follow-up milestones are met
  • Identify care bottlenecks and gaps and propose process improvements
  • Serve as a liaison for patients, families, caregivers, staff, and referring physicians
  • Provide psychosocial screening and refer patients to social workers, financial counselors, and support services
  • Support shared decision-making, advance care planning, and palliative care discussions
  • Attend Tumor Board Conference and coordinate recommended care
  • Maintain communication with the tumor registry
  • Solicit feedback from referring physicians and develop clinical operations improvements
  • Develop patient education materials and collaborative community relationships
  • Participate in cancer education, prevention, outreach, performance improvement, and program development
  • Mentor and orient healthcare professionals and new staff
  • Maintain professional knowledge through continuing education, conferences, seminars, workshops, and professional organizations
  • Ensure documentation, policy compliance, and safeguarding of University assets

Requirements

What you’ll need
  • Bachelor’s Degree
  • Registered Nurse Licensing (RN)
  • Minimum 2-3 years of relevant work experience
  • Ability to maintain effective interpersonal relationships
  • Ability to communicate effectively in both oral and written form
  • Skill in collecting, organizing and analyzing data
  • Proficiency in computer software, including Microsoft Office
  • Proficiency in Electronic Medical Record system(s)
  • Ability to perform duties as an independent professional and team player
  • Organizational skills sufficient to set priorities and facilitate work team progress
  • Compliance with mandatory in-services and employee health procedures
  • Compliance with facility policies and procedures and HIPPA, ACHA, JCAHO, OSHA standards and regulations
