About the role

Key responsibilities & impact
  • Assist in patient management in all phases of care as a member of the solid organ transplant multidisciplinary care management team
  • Assess the psychological, financial, and discharge planning needs of patients and families
  • Provide information, support, counseling, and referrals to other levels of care and appropriate resources
  • Support transplant patients and a variety of age-specific populations, including neonatal, pediatric, adolescent, adult, and geriatric populations
  • Apply Social Work scope of practice, current state requirements, CMS Conditions of Participation, EMTALA, Patient Bill of Rights, AB 1203, and other applicable federal or state regulatory requirements
  • Partner with the multidisciplinary team using best practices, relevant data, and UAMS mission, standards, goals, and core values

Requirements

What you’ll need
  • Master's degree in Social Work
  • Must be licensed by an Arkansas Professional Board as a Licensed Masters Social Worker (LMSW) by date of hire
  • Proof of legal authority to work in the United States on the first day of employment
  • Adult Maltreatment and annual TB screening
  • Subject to criminal background, drug testing, and education verification
  • Preferred: experience in case management
  • Preferred: experience working in a medical setting
  • Preferred: Certified Case Manager (CCM) or American Case Management (ACM) certification
  • Preferred: experience working with the transplant patient population
