About the role

Key responsibilities & impact
  • Provide direct patient care as an RN.
  • Coordinate care for patients and triage patients.
  • Work collaboratively with clinic providers, fellows, and staff.
  • Facilitate appointments, coordinate transitions of care, and provide patient education on complex conditions.
  • Develop and implement individual health plans in collaboration with patients, their families, and providers of care.
  • Perform comprehensive nursing assessments and identify patient health needs.
  • Develop and implement individualized care plans in collaboration with physicians, nurse practitioners, and other healthcare providers.
  • Coordinate patient care, including referrals, follow-up appointments, diagnostic testing, and continuity of care.
  • Educate patients and families on disease prevention, health maintenance, medications, treatment plans, and self-management strategies.
  • Monitor patient progress, document assessments and interventions in electronic medical record, and communicate changes in patient status to providers.

Requirements

What you’ll need
  • Baccalaureate Degree BSN, ADN Degree, or Diploma RN. Major: Registered Nurse.
  • RN with a minimum of 1 year of RN experience.
  • Current BLS certification.
  • ACLS certification or obtain certification within one year of hire date preferred.
  • Knowledge or experience in PCMH desired.
