About the role
Key responsibilities & impact- Provide direct patient care as an RN.
- Coordinate care for patients and triage patients.
- Work collaboratively with clinic providers, fellows, and staff.
- Facilitate appointments, coordinate transitions of care, and provide patient education on complex conditions.
- Develop and implement individual health plans in collaboration with patients, their families, and providers of care.
- Perform comprehensive nursing assessments and identify patient health needs.
- Develop and implement individualized care plans in collaboration with physicians, nurse practitioners, and other healthcare providers.
- Coordinate patient care, including referrals, follow-up appointments, diagnostic testing, and continuity of care.
- Educate patients and families on disease prevention, health maintenance, medications, treatment plans, and self-management strategies.
- Monitor patient progress, document assessments and interventions in electronic medical record, and communicate changes in patient status to providers.
Requirements
What you’ll need- Baccalaureate Degree BSN, ADN Degree, or Diploma RN. Major: Registered Nurse.
- RN with a minimum of 1 year of RN experience.
- Current BLS certification.
- ACLS certification or obtain certification within one year of hire date preferred.
- Knowledge or experience in PCMH desired.
