About the role

Key responsibilities & impact
  • Provide compassionate and skilled oncology expertise spanning the entire cancer care continuum; including diagnosis, treatment, survivorship, and end of life care.
  • Provide assistance to patients, families and care givers to help overcome healthcare system barriers and facilitate timely access to quality health and psychosocial care.
  • Evaluate the needs and coordinate the care of the patient through the continuum of care by ensuring patient needs are met.
  • Facilitate patient care coordination with interdisciplinary teams within internal departments and external service lines.
  • Provide education to patients and caregivers, supporting compliance with appointments and treatments.
  • Collaborate in interdisciplinary tumor board.
  • Assess education needs and seek to update knowledge base to provide quality and current patient specific specialty service line coordination.

Requirements

What you’ll need
  • Associate’s degree in nursing, Diploma RN, Bachelor’s Degree in Nursing or Master’s degree in Nursing plus three (3) years Registered Nurse experience.
  • Graduate of an accredited RN nursing program.
  • Valid Registered Nursing License
  • Basic skills with Microsoft programs, Word, Excel, PowerPoint
  • Excellent communication skills.
  • Strong work ethics and accountability.
  • Minimum of one (1) year of relevant Registered Nursing experience in Oncology or Minimum of five (5) years in patient navigation/coordination
  • Current BLS National Certification – AONN or OCN or Oncology related certification
  • Knowledgeable with EPIC EMR
