About the role

Key responsibilities & impact
  • Promote wellness; improve outcomes, and self-care within a defined population of patients
  • Provide patients who have chronic conditions with support, education, and assistance in prevention and/or maintenance of their disease and/or health and wellness state
  • Work with individual patients, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self-management
  • Coordinate care and/or follow-up for patients who receive care in outpatient facilities
  • Coordinate prescription refill requests and patient education in collaboration with physician staff
  • Collect and document assessment data from patients and/or others per UAMS standards
  • Develop patient's Plan of Care (POC) per department/unit standards utilizing assessment data
  • Administer medication following department/unit standards and Nurse Practice Act
  • Support the integration of current evidence into nursing practices
  • Provide effective patient/family education/discharge planning
  • Collaborate and communicate with nursing staff at all levels, interdisciplinary teams, executive leaders, and other stakeholders

Requirements

What you’ll need
  • Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing
  • Valid RN License
  • One (1) year Registered Nursing experience
  • Excellent communication skills
  • Basic computer skills
  • Strong work ethic and accountability
  • Nursing experience in ICU or ER (preferred)
  • Current BLS and ACLS Certification (preferred)
