About the role

Key responsibilities & impact

Promote wellness; improve outcomes, and self-care within a defined population of patients
Provide patients who have chronic conditions with support, education, and assistance in prevention and/or maintenance of their disease and/or health and wellness state
Work with individual patients, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self-management
Coordinate care and/or follow-up for patients who receive care in outpatient facilities
Coordinate prescription refill requests and patient education in collaboration with physician staff
Collect and document assessment data from patients and/or others per UAMS standards
Develop patient's Plan of Care (POC) per department/unit standards utilizing assessment data
Administer medication following department/unit standards and Nurse Practice Act
Support the integration of current evidence into nursing practices
Provide effective patient/family education/discharge planning
Collaborate and communicate with nursing staff at all levels, interdisciplinary teams, executive leaders, and other stakeholders

Requirements

What you’ll need

Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing
Valid RN License
One (1) year Registered Nursing experience
Excellent communication skills
Basic computer skills
Strong work ethic and accountability
Nursing experience in ICU or ER (preferred)
Current BLS and ACLS Certification (preferred)
