About the role

Key responsibilities & impact
  • Provides supportive services to family and care team members throughout the patient's assessment and treatment
  • Conducts comprehensive psychosocial assessments to identify barriers to health
  • Provides brief interventions, counseling, crisis intervention, and supportive services for patients and families within the primary care setting
  • Coordinates referrals to community resources, social service agencies, behavioral health providers, and specialty care
  • Assists patients in accessing benefits and services, including Medicaid, Medicare, disability resources, and community assistance programs
  • Collaborates with physicians, nurse practitioners, nurses, pharmacists, psychologists, and other members of the interdisciplinary care team to develop patient-centered care plans
  • Participates in population health initiatives that improve health outcomes for patients with chronic diseases and complex medical and social needs
  • Supports care transitions by helping patients navigate hospital discharges, follow-up appointments, and continuity of care
  • Documents patient encounters and interventions in the electronic medical record while maintaining compliance with UAMS policies and HIPAA requirements
  • Educates patients and families about coping strategies, disease management, and available community resources
  • Participates in quality improvement projects, program development, and team meetings to enhance integrated primary care services
  • May perform other duties as assigned

Requirements

What you’ll need
  • Bachelor degree in social work, counseling community health or related field
  • One (1) year of experience in care and resource coordination for clients in medical settings
  • Preferred Qualifications: LSW
