RN Care Manager

About the role
Key responsibilities & impact- Managing the continuum of care for admission through discharge for assigned patients
- Reflecting appropriate knowledge of the RN scope of practice and current state requirements
- Partnering with the multi-disciplinary team and utilizing scientific evidence for best practices
- Coordinating the discharge planning process in collaboration with the interdisciplinary team
- Documenting all communication and concerns pertaining to coordinating care and services
- Communicating with physicians regarding level of care or admission status when criteria are not met
- Identifying patients/families with psychosocial and/or non-medical discharge planning needs
- Assisting in Performance Improvement, Evidenced Based Practice/Research process
Requirements
What you’ll need- Baccalaureate Degree in Nursing, ADN, or Diploma RN Degree
- Three (3) years RN experience with current BLS certification
- Valid Arkansas Registered Nursing License
- National Nursing Certification preferred; BSN preferred; Certified Case Manager (CCM) or American Case Management (ACM) certification
