About the role
Key responsibilities & impact- Provide patient-facing care in clinic for 2 days per week
- Conduct telehealth visits and in-basket coverage for 3 days per week
- Manage acute and chronic conditions across the lifespan
- Perform preventive care visits
- Order and interpret diagnostic testing
- Develop treatment plans
- Prescribe medications within scope of practice
- Document care in the electronic medical record
- Communicate effectively with patients, families, physicians, and the broader care team
Requirements
What you’ll need- Board certified
- Eligible for licensure in the State of New York
- 2-3+ years of experience as a Nurse Practitioner in family medicine, primary care, pediatrics, internal medicine, or a related outpatient setting
- Master of Science in Nursing (MSN) degree
- Possession of an NPI (National Provider Identification) number
- Successful completion of an approved Nurse Practitioner program
- Current NYS Registered Professional Nurse License
- Certification from American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP)
- Current NYS Certified Nurse Practitioner (CNP) license and certification with New York State Education Department
- Documentation of current CPR certification within one (1) week of hire,
- Certified Prescriptive Privilege in NYS
- Advanced Cardiovascular Life Support (ACLS) Certification is preferred
- Possession of Bureau of Narcotics and Dangerous Drugs (BNDD) Number is preferred
