Key responsibilities & impact

Support high-quality, patient-centered acute care through clinical expertise.
Screen ED and inpatient candidates for eligibility and support timely enrollment.
Coordinate seamless transition to home-based care.

Represent the team in interdisciplinary case conferences, partnering with physicians, nursing, and care management.

Requirements

What you’ll need

Associate’s Degree in a Human Service-related field.
May accept 5+ years of related care coordination experience in lieu of an Associate's Degree.
Preferred: Bachelor’s Degree in Science or Arts in Human Services related field.
Minimum Required: LPN certification.
