About the role
- Conduct virtual and home visits, as well as visits at Unimed facilities
- Monitor the clinical care process
- Promote routines aligned with a culture of quality and patient safety
- Audit and evaluate processes with a focus on continuous improvement
- Monitor patients with chronic diseases and/or other conditions
- Record interventions and patient progress in designated systems/platforms
- Oversee the implementation of action plans and care plans for approved projects
- Generate and analyze program indicators, proposing improvements and action plans
- Manage individual patient care plans
Requirements
- Bachelor’s degree in Nursing
- Experience in oncology
- Experience in palliative care
Benefits
- Availability to work 220 hours per month
- Proof of occupational vaccinations required as specified in the PCMSO (Occupational Health Medical Control Program)
