About the role
Key responsibilities & impact- Manage a designated caseload to coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans and care transitions.

Requirements
What you’ll need- Massachusetts RN Licensure.
- 3 Years of RN Experience.
- Current Basic Life Support (BLS) Certification.
- Bachelor of Science in Nursing (BSN) preferred.
