About the role

Key responsibilities & impact
  • Provide extensive nutritional and lifestyle education to patients via telehealth
  • Help patients achieve and maintain healthy weight loss
  • Guide patients to appropriate community resources
  • Assess nutritional and lifestyle outcomes and develop reports tracking progress
  • Formulate and communicate clinical recommendations and visit summaries to healthcare providers and patients
  • Identify and address barriers to effective patient care
  • Partner with departments to leverage technology and health information for better patient outcomes
  • Deliver clinical reports and presentations with the account manager
  • Educate clients on benefit plans, clinical programs, and solutions
  • Increase adoption and implementation of clinical programs
  • Plan, organize, and direct responsibilities delegated by the Clinical Innovation Pharmacist

Requirements

What you’ll need
  • Bachelor’s degree required
  • Registered Dietitian Certification (CDR)
  • State Certification or Licensure
  • Strong work experience counseling individuals in an outpatient setting with complex disease states to prevent complications
  • Familiarity with current treatment standards and options for common metabolic conditions, including diabetes and obesity
  • Excellent verbal and written communication skills
  • Proficiency in MS Word, Excel, and Outlook
  • Experience creating and updating Excel spreadsheets
  • Ability to work independently with minimal supervision
  • Skilled in identifying patient diet/lifestyle barriers and developing strategies to overcome them
  • Significant experience in patient education and engagement
  • Willingness to travel as required
