About the role
- Serve as the primary nurse coordinating care for your assigned patients
- Develop, implement, and update individualized plans of care
- Determine visit frequency based on patient acuity and family needs
- Provide guidance to Hospice Aides and LPNs to ensure excellent care
- Communicate changes in condition to physicians and the care team
- Participate in on-call rotation
Requirements
- Active RN license in the state you’ll practice
- Graduate of an accredited nursing program (ADN or BSN)
- 2+ years of RN experience preferred (hospice experience required)
- Strong clinical judgment, compassion, and communication skills
- Reliable transportation, valid driver’s license, auto insurance, and CPR certification
