Inpatient Clinical Pharmacist – Third Shift
Trinity HealthInpatient Clinical Pharmacist responsible for optimizing medication therapy and providing clinical services in a healthcare team. Collaborating with healthcare professionals for patient-centered pharmaceutical care.
About the role
Key responsibilities & impact- Develop, recommend, and monitor individualized pharmacotherapeutic regimens using expertise in pharmacology, pathophysiology, and pharmacokinetics.
- Participate in patient care rounds and collaborate closely with physicians, nurses, and other healthcare professionals to ensure optimal medication outcomes.
- Offer timely drug information, therapeutic recommendations, and evidence‑based guidance to the medical staff.
- Support antimicrobial stewardship, anticoagulation management, and other clinical initiatives as assigned.
- Provide overnight staffing support in the inpatient central pharmacy during scheduled third‑shift hours (generally 9:00 p.m. – 7:00 a.m.).
- Operate on a rotating 7‑on/7‑off schedule in coordination with other third-shift pharmacists; shift hours may be adjusted based on departmental and operational needs.
- Responsibilities include medication order verification, medication distribution, and sterile and non‑sterile compounding in compliance with hospital policies, regulatory requirements, and accreditation standards.
- Accurately compound, package, label, and dispense medications, IV solutions, and specialized preparations.
- Ensure compliance with USP / and maintain a high standard of safety and quality.
- Provide oversight, guidance, and support to pharmacy technicians and pharmacy interns in daily operations.
- Ensure appropriate task delegation and adherence to workflow standards.
Requirements
What you’ll need- Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) or other advanced degree preferred.
- Must be licensed, or eligible for licensure, as a pharmacist in the State of Michigan.
- Strongly preferred: completion of an ASHP‑accredited residency (PGY‑1 and/or PGY‑2)
- Demonstrated ability to interact effectively and professionally with patients, families, healthcare team members, support staff, medical staff, and the public.
- Strong verbal and written communication skills with the ability to convey clinical information clearly and compassionately.
- Proven ability to perform all duties in a safe, accurate, and compliant manner while adhering to hospital policies, procedures, and regulatory standards in the delivery of patient care.
- Ability to identify medication‑related issues and patient care concerns, recognize situations outside the pharmacist’s scope of responsibility, and initiate appropriate follow‑up or escalation.
- Demonstrated flexibility in adjusting work priorities in collaboration with supervisors based on patient care needs, staffing demands, or departmental changes.
- Ability to thrive in a dynamic, fast‑paced environment requiring significant adaptability.
- Ability to clearly articulate and consistently demonstrate a commitment to the philosophy, values, mission, and vision of Trinity Health.
- Demonstrated dedication to contemporary hospital pharmacy practice, evidenced by recent (within the past 5 years) inpatient pharmacy experience, residency training, or advanced clinical practice.
