About the role
- responsible for client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes
- collaborating with the physician and other Hospice team members
- anticipate and manage patient’s symptoms
- assists in identifying pt/family/caregiver grief/loss issues
- assists in implementing the Hospice plan of care
Requirements
- Active RN license in Michigan
- 1+ years of clinical nursing experience (Hospice preferred)
- Strong communication, assessment, and organizational skills
- Compassionate, dependable, and mission-minded
- Valid driver’s license and reliable transportation required
