About the role

Key responsibilities & impact
  • Provide direct supervision and oversight for a localized, Integrated Care Team supporting Innovations members within a designated region
  • Provide support, education, and guidance to staff as needed
  • Develop, implement, and monitor standards, procedures, and processes with approval of the Operations Director and Senior Director of IDD Care Management
  • Provide leadership with current programs or projects as assigned
  • Conduct research or needs assessments or attend training programs to assess opportunities for Trillium, and assist with implementation
  • Review dashboard metrics to ensure compliance with Medicaid, HCBS, and Innovations Waiver requirements
  • Monitor critical incidents and escalated concerns
  • Resolve member and provider concerns
  • Oversee an integrated team providing localized support in a managed care environment
  • Ensure implementation of industry-standard, evidence-based best practices and whole-person care

Requirements

What you’ll need
  • Bachelor's degree in a health or human services field
  • Three (3) years of progressive experience in care management, case management, or care coordination with MH/SU/DD populations
  • Experience in program development and/or program management
  • Must have a valid driver's license
  • Must reside within North Carolina
  • Must be able to travel within catchment as required
  • Experience supervising a team of two or more employees preferred
  • Experience working with Innovations in mental health, substance use, intellectual/developmental disability, and/or TBI populations preferred
  • Experience in Innovations Waiver, HCBS, Care Management, and Managed Care preferred
  • Must pass a drug test as a condition of employment
