About the role

Key responsibilities & impact

Provide direct supervision and oversight for a localized, Integrated Care Team supporting Innovations members within a designated region
Provide support, education, and guidance to staff as needed
Develop, implement, and monitor standards, procedures, and processes with approval of the Operations Director and Senior Director of IDD Care Management
Provide leadership with current programs or projects as assigned
Conduct research or needs assessments or attend training programs to assess opportunities for Trillium, and assist with implementation
Review dashboard metrics to ensure compliance with Medicaid, HCBS, and Innovations Waiver requirements
Monitor critical incidents and escalated concerns
Resolve member and provider concerns
Oversee an integrated team providing localized support in a managed care environment
Ensure implementation of industry-standard, evidence-based best practices and whole-person care

Requirements

What you’ll need

Bachelor's degree in a health or human services field
Three (3) years of progressive experience in care management, case management, or care coordination with MH/SU/DD populations
Experience in program development and/or program management
Must have a valid driver's license
Must reside within North Carolina
Must be able to travel within catchment as required
Experience supervising a team of two or more employees preferred
Experience working with Innovations in mental health, substance use, intellectual/developmental disability, and/or TBI populations preferred
Experience in Innovations Waiver, HCBS, Care Management, and Managed Care preferred
Must pass a drug test as a condition of employment
