About the role
Key responsibilities & impact- Provide direct supervision and oversight for a localized, Integrated Care Team supporting Innovations members within a designated region
- Provide support, education, and guidance to staff as needed
- Develop, implement, and monitor standards, procedures, and processes with approval of the Operations Director and Senior Director of IDD Care Management
- Provide leadership with current programs or projects as assigned
- Conduct research or needs assessments or attend training programs to assess opportunities for Trillium, and assist with implementation
- Review dashboard metrics to ensure compliance with Medicaid, HCBS, and Innovations Waiver requirements
- Monitor critical incidents and escalated concerns
- Resolve member and provider concerns
- Oversee an integrated team providing localized support in a managed care environment
- Ensure implementation of industry-standard, evidence-based best practices and whole-person care
Requirements
What you’ll need- Bachelor's degree in a health or human services field
- Three (3) years of progressive experience in care management, case management, or care coordination with MH/SU/DD populations
- Experience in program development and/or program management
- Must have a valid driver's license
- Must reside within North Carolina
- Must be able to travel within catchment as required
- Experience supervising a team of two or more employees preferred
- Experience working with Innovations in mental health, substance use, intellectual/developmental disability, and/or TBI populations preferred
- Experience in Innovations Waiver, HCBS, Care Management, and Managed Care preferred
- Must pass a drug test as a condition of employment
