About the role
- Intensive medical monitoring and basic and therapeutic nursing care in the home environment
- Point of contact for the patient and their relatives
- Communication and coordination with therapists
- Implementation of rehabilitative measures and social support
- Outings and activities with the patient and, if applicable, their relatives
- Maintain nursing documentation
Requirements
- Qualified as a geriatric nurse, registered nurse, pediatric nurse, or specialist in out-of-hospital (home) intensive care
- Driver's license
- Interest in outpatient care for patients requiring intensive nursing
- Passion for one-to-one nursing care
- Conscientious, independent, and responsible working style
- Empathy and compassionate patient care
