About the role
- Intensive medical monitoring, basic and treatment nursing care in the home environment
- Transfers using assistive devices
- Monitoring of enteral nutrition via PEG
- Administration of medication as prescribed by a physician
- Point of contact for the patient and their relatives
- Coordination and contact with therapists
- Implementation of rehabilitative measures and provision of social support
- Outings and excursions with patients and, if applicable, relatives
- Maintaining nursing documentation
Requirements
- Qualified geriatric nurse or registered nurse, nursing specialist, or pediatric nurse
- Alternatively: Specialist in out-of-hospital intensive care, nursing expert for out-of-hospital intensive and ventilator care
- Specialist nurse in intensive care/anesthesia or respiratory therapist
- Interest in outpatient care for patients requiring intensive care
- Passion for one-to-one care
- Conscientious, independent, and responsible working style
- Empathy and a caring approach to patients
