About the role
- Conduct regular home or facility visits to assess client needs (medical, social, functional and psychological)
- Create and implement care plans tailored to each client's unique situation
- Coordinate services and care providers, and advocate for the client's best interest
- Communicate regularly with families and care partners to ensure high-quality, person-centered care
- Accompany clients to medical appointments and other activities as needed
- Work independently with ongoing support from a collaborative team
- Provide personalized consultation and care management services to support older adults and their families
Requirements
- Nurse, social worker, or gerontologist (profession required)
- Passionate and experienced professionals (experience expected)
- Ability to conduct regular home or facility visits and accompany clients to appointments
- Ability to create and implement individualized care plans
- Ability to coordinate services and care providers and advocate for clients
- Communicate regularly with families and care partners
- Compliance with California fingerprint identification and background requirements (for California applicants)
