About the role
Key responsibilities & impact- Manage patient flow, triage, and direct patient care
- Coordinate communication among front desk staff, nursing staff, physicians, and the clinic manager
- Develop expected patient outcomes, define resource utilization, and establish patient goals with providers
- Provide nursing support and care coordination through clinical and educational programs and support services
- Support patient self-management of disease
- Coordinate continuity of care after hospital admissions, discharges, and emergency room visits
- Manage high-risk patients, including those with multiple comorbidities or high readmission risk, using a patient registry
- Assess patients' physical, psychosocial, financial, and environmental healthcare needs
- Educate patients and ancillary personnel on healthcare needs, safety issues, medications, diagnostic procedures, nutrition, and wellness
- Document assessments, treatment plans, clinical findings, and progress in the EMR
- Observe, record, and report patient conditions and reactions to drugs and treatments
- Maintain and review patient records and follow up on tests and examination results
- Respond to and refer patient treatment-related phone calls
- Maintain patient confidentiality
Requirements
What you’ll need- High school graduate and bachelor's degree in nursing listed under required education and experience
- 2 or more years of RN experience
- Current active multistate Registered Nurse (RN) license in good standing in the state where the candidate lives
- Basic Cardiac Life Support (BLS/BCLS) certification from the American Heart Association within 14 days
- Basic typing and word processing ability
- Excellent analytical, critical-thinking, and problem-solving skills
- Must perform the professional, clinical, and/or technical competencies of the assigned unit or department
- Previous medical office experience preferred
