Support the Home First team with assessments, reviews and interventions.
Work alongside customers to identify strengths, goals and outcomes that promote independence and wellbeing.
Build positive, trusting relationships with customers, their families and carers to deliver personalised support.
Collaborate with social care colleagues, health professionals and voluntary sector partners to provide joined-up, integrated care.
Contribute to a service that focuses on prevention, recovery and enabling people to live safely and independently at home for as long as possible.

Requirements

What you’ll need

A recognised Occupational Therapy qualification and current HCPC registration.
Excellent communication and relationship-building skills when working with customers, families, carers, advocates and professional partners.
The ability to respond effectively to urgent priorities within a fast-paced, team-based environment.
Resilience, adaptability and a positive approach when managing complex situations and competing demands.
