About the role

Key responsibilities & impact
  • Lead and manage the Integrated Neighbourhood Team and associated Connect services, helping to deliver coordinated, person-centred care within the local community.
  • Provide strong, visible leadership to the Integrated Neighbourhood Team.
  • Responsible for clinical and care governance, including the setting and monitoring of standards and performance.
  • Oversee the local implementation of health and care strategies, working closely with a wide range of partners including primary care, acute and specialist services, mental health services, housing, the voluntary and community sector, and care providers.

Requirements

What you’ll need
  • Significant experience managing services and teams within a health and/or social care environment, including leading multi‑disciplinary or integrated teams.
  • Strong leadership and partnership‑working skills, with the ability to work collaboratively across health, social care, voluntary and community sector organisations.
  • Proven ability to manage performance, quality and risk, using data and professional judgement to drive improvement and positive outcomes.
  • Experience of managing and being accountable for large or complex budgets, demonstrating value for money and effective use of resources.
  • Excellent communication and influencing skills, with the ability to engage and negotiate with staff, partners, senior leaders and external stakeholders.
