About the role

Key responsibilities & impact
  • Manage a portfolio/caseload of low- to moderate-acuity clients throughout their healthcare journeys
  • Educate clients, advocate on their behalf, coordinate care, and provide ongoing support
  • Build trusted relationships with clients and serve as a reliable healthcare guide
  • Help clients access appropriate providers, facilities, and services
  • Gather and synthesize clinical information and prepare summaries for internal teams
  • Coordinate client handoffs and continuity across clinical and operational teams
  • Research and recommend providers and facilities based on client needs
  • Coordinate appointments, records, referrals, and other care logistics
  • Communicate with clients, caregivers, providers, and internal partners
  • Collaborate cross-functionally to deliver a seamless client experience

Requirements

What you’ll need
  • Active Registered Nurse license in the state of residence
  • At least five years of healthcare experience
  • Experience in case management, care coordination, patient navigation, or a related clinical environment
  • Ability to consistently work Monday through Friday, 8:30 a.m. to 5:00 p.m. Pacific or Mountain Time
  • Experience working remotely
  • Confidence, empathy, and sound clinical judgment
  • Strong written and verbal communication skills
  • Ability to translate complex clinical information for clinical and non-clinical audiences
  • Self-directed, collaborative approach
  • Strong organization and time management
  • Comfort learning new technologies and systems
