About the role
Key responsibilities & impact- Manage a portfolio/caseload of low- to moderate-acuity clients throughout their healthcare journeys
- Educate clients, advocate on their behalf, coordinate care, and provide ongoing support
- Build trusted relationships with clients and serve as a reliable healthcare guide
- Help clients access appropriate providers, facilities, and services
- Gather and synthesize clinical information and prepare summaries for internal teams
- Coordinate client handoffs and continuity across clinical and operational teams
- Research and recommend providers and facilities based on client needs
- Coordinate appointments, records, referrals, and other care logistics
- Communicate with clients, caregivers, providers, and internal partners
- Collaborate cross-functionally to deliver a seamless client experience
Requirements
What you’ll need- Active Registered Nurse license in the state of residence
- At least five years of healthcare experience
- Experience in case management, care coordination, patient navigation, or a related clinical environment
- Ability to consistently work Monday through Friday, 8:30 a.m. to 5:00 p.m. Pacific or Mountain Time
- Experience working remotely
- Confidence, empathy, and sound clinical judgment
- Strong written and verbal communication skills
- Ability to translate complex clinical information for clinical and non-clinical audiences
- Self-directed, collaborative approach
- Strong organization and time management
- Comfort learning new technologies and systems
