About the role
Key responsibilities & impact- Conduct regular video and/or telephonic appointments with clients seeking mental health support
- Establish positive and trusting relationships with clients
- Screen and assess patients for mental health and substance use disorders using evidence-based tools
- Provide patient education about disorders and evidence-based treatments
- Implement treatments and protocols using evidence-based techniques
- Continually assess client situations and provide ongoing treatment
- Educate and involve family members or loved ones in treatment when necessary
- Partner with plan and community resources to make referrals to outside sources or agencies
- Coordinate care after acute events, including emergency department presentations or inpatient admissions
- Coordinate with the patient’s care plan team to ensure holistic care
- Maintain client confidentiality
- Participate in assigned clinical team meetings
Requirements
What you’ll need- LCSW, LPC, or LMFT
- Current, unrestricted license(s) in New York
- Independent license required
- At least 2 years of therapy experience
- Bilingual Spanish and ability to pass language testing
- Experience with population management
- Ability to practice in the office in New York
- Home internet through cable broadband or fiber optic service
- Minimum internet speeds of 10 Mbps download and 5 Mbps upload when working from home
