About the role

Key responsibilities & impact
  • Support patients transitioning from hospital to home by coordinating specialty and home infusion services.
  • Collaborate with hospitals, clinics, physicians, and home health agencies to ensure timely and effective home-based care.
  • Manage the full referral lifecycle, including insurance verification, home health scheduling, and coordination with operational teams.
  • Educate patients and caregivers on therapy expectations and financial responsibilities with empathy and clarity.
  • Build and maintain strong relationships with referral sources while supporting growth across assigned hospitals and clinics.
  • Identify patient and family concerns and assist with issue resolution to promote satisfaction and continuity of care.
  • Maintain accurate documentation and ensure patient confidentiality at all times.
  • Participate in required meetings, in-services, and training activities.
  • Comply with established administrative, HR, and organizational policies.

Requirements

What you’ll need
  • Strong customer focus with the ability to identify and respond to patient and referral source needs.
  • Excellent communication and interpersonal skills.
  • Strong organizational, analytical, and problem-solving abilities.
  • Ability to manage multiple priorities and work independently in a fast-paced environment.
  • Proficiency in Microsoft Word and Excel.
  • Bachelor’s degree or equivalent experience (preferred).
  • Experience in managed care, insurance, or healthcare coordination (preferred).
  • Project management or public-speaking experience (preferred).
  • Ability to collaborate with diverse stakeholders across healthcare settings (preferred).
