About the role

Key responsibilities & impact
  • Manage all aspects of program services according to Independence & Mobility Program guidelines and SOPs
  • Collaborate with peers and management on program development, implementation, monitoring, tracking, data collection, action plans, program deliverables, and stakeholder communication
  • Conduct virtual home assessments evaluating accessibility, safety, and functional use of entrances, exits, bathrooms, bedrooms, kitchens, and pathways
  • Identify environmental barriers and assess current and future accessibility needs, including mobility, transfers, activities of daily living, emergency preparedness, and evacuation planning
  • Educate individuals and caregivers on accessibility solutions, safety strategies, future planning, funding, and equipment resources
  • Develop individualized recommendations and comprehensive written reports covering home modifications, adaptive equipment, assistive technology, and accessibility solutions
  • Respond to follow-up questions and provide additional consultation
  • Maintain accurate assessment records and status updates in ClickUp
  • Track requests, appointments, report completion, and program metrics
  • Provide clients with program evaluation surveys and measure outcomes
  • Perform other related duties assigned by leadership

Requirements

What you’ll need
  • Bachelor’s degree or equivalent education + experience
  • Knowledge of durable medical equipment and assistive technology required
  • Excellent organization skills and attention to detail
  • Comfortable working in a database system
  • Proficiency in Microsoft Office Suite
  • Ability to work independently with minimal supervision; must be a self-starter
  • Ability to manage multiple projects simultaneously
  • Excellent customer service skills
  • Assistive Technology Professional (ATP), Occupational Therapist (OT), Physical Therapist (PT), or Speech Language Pathologist (SLP) certification/licensure helpful
  • Knowledge of equipment types and variety as related to the progression of individuals living with ALS preferred
  • Salesforce and database experience a plus
