About the role

Key responsibilities & impact

Conduct community outreach and health screenings
Support Medicaid members and individuals with asthma or chronic health conditions
Complete health assessments and questionnaires
Conduct home visits, approximately 8 per month, with some virtual visits
Connect community members with resources and health-related services
Assist with environmental health initiatives and supply coordination
Present educational workshops and community programs
Maintain accurate records across multiple systems

Requirements

What you’ll need

Fluent in Spanish and English (Required)
Must live in Staten Island, NY or within a few mile radius
Passion for community health and helping underserved populations
Ability to work independently and stay organized
Comfortable with outreach, home visits, and public presentations
Tech-savvy and able to learn multiple systems quickly
Healthcare, social services, community outreach, or customer-facing experience preferred
