About the role
Key responsibilities & impact- Manage accounts using the 4-Prong approach, meeting expected billable hours and using Conifer Value Based Care tools according to standard protocol
- Document Status, Sub-status, Patient Severity, and Medical Management Acuity accurately
- Review the daily calendar for new episodes and due tasks and prioritize activities
- Complete Personal Health Management assessments using clinical claim charts, member discussions, and physician assessment and treatment plans
- Identify and document member goals and engage members in the Personal Health Management process
- Develop and maintain individualized plans of care with timeframes, expected outcomes, interventions, and member goals
- Follow through on interventions and document outcomes in the Current Clinical Status section of the care plan
- Engage PCPs and treating physicians in the Personal Health Management process
- Collaborate with members and PCPs to develop individualized plans of care
- Close episodes as indicated, including evaluating impactability
- Interface with clients and CCCS client managers regarding medical management of designated clients
- Perform Personal Health Management and some Utilization Management functions
- Travel approximately 5–30% depending on the assigned client
Requirements
What you’ll need- Bachelor’s degree in a health-related field
- Three to five years of diverse clinical experience required
- Five years of UM/CM experience preferred
- RN (Registered Nurse) license required
- Current active professional license in the state of residence
- Eligibility for unrestricted professional licenses in all states
- CCM preferred
- Understanding of holistic medical management and client needs
- Understanding of legislative acts such as the ADA
- Competency in Microsoft Word and email with attachments
- Excellent verbal and written professional communication skills
- Ability to analyze and resolve complex problems
- General understanding of cost containment and its relationship to medical management
- Excellent organizational, prioritization, and time management skills
- Ability and willingness to work independently and as part of a team
- Ability to work seated, use a computer, and answer the telephone
- Ability to travel and walk through hospital-based departments, including Emergency Department environments
- Required vaccinations and screenings may need to be obtained and confirmed before employment, as applicable and permitted by law
