About the role

Key responsibilities & impact
  • Proactively contact identified members to drive behavior change and positively impact clinical outcomes
  • Engage members by telephone, email, text, virtual application, and coaching conversations
  • Provide feedback and support, answer questions, share resources, and hold members accountable to their goals
  • Use a coaching approach congruent with motivational interviewing to empower members to self-manage chronic conditions and health
  • Refer members for condition-specific support and education based on clinical care guidelines
  • Maintain accurate and thorough case documentation according to Teladoc Health standards
  • Complete administrative duties essential for smooth operation
  • Support corporate initiatives and special projects as needed

Requirements

What you’ll need
  • Bachelor’s Degree in relevant discipline or MA/EMT/Paramedic certification
  • 2+ years experience in a healthcare delivery or related environment
  • Excellent written and verbal communication skills
  • Exceptional time management and organizational skills
  • Analytical and critical thinking skills
  • Ability to work autonomously in a responsible, efficient manner
  • Excellent customer service and interpersonal skills
  • Flexibility to work in a fast-paced environment
  • Adaptability and willingness to embrace teamwork and change
  • Ability to learn and effectively use multiple technology platforms, including MS Word, Excel, PowerPoint, Outlook 365, Microsoft Teams, SharePoint, OneDrive, Salesforce, and Jira
  • Bilingual Spanish and English preferred
  • Currently authorized to work in the United States without visa sponsorship now or in the future
