About the role
Key responsibilities & impact- Provide specialist pharmaceutical support to patients in collaboration with NHS Partners
- Conduct reviews for patients with long-term conditions within clinical competencies
- Manage chronic disease management and structured medication reviews
- Actively participate in quality improvement and audits
- Collect and record patients’ health data holistically
- Support medicines management and monitoring as per guidelines
- Offer evidence-based lifestyle advice and prescribing medications
Requirements
What you’ll need- Minimum of 12 months experience as a clinical pharmacist within an NHS Primary Care setting
- Current member of a medical professional defense organisation
- Independent prescriber with completed GPhC-accredited course/or equivalent
- Currently registered with the GPC and compliant with its code of Professional Conduct
- Experience managing practice medicines management workflow
- Excellent communication skills, both written and spoken
- IT literate, proficient with software systems like Emis Web, SystmOne, and Microsoft Office
- Confident in managing conditions like Hypertension, Type 2 Diabetes, Asthma, and more
