About the role

Key responsibilities & impact

Provide specialist pharmaceutical support to patients in collaboration with NHS Partners
Conduct reviews for patients with long-term conditions within clinical competencies
Manage chronic disease management and structured medication reviews
Actively participate in quality improvement and audits
Collect and record patients’ health data holistically
Support medicines management and monitoring as per guidelines
Offer evidence-based lifestyle advice and prescribing medications

Requirements

What you’ll need

Minimum of 12 months experience as a clinical pharmacist within an NHS Primary Care setting
Current member of a medical professional defense organisation
Independent prescriber with completed GPhC-accredited course/or equivalent
Currently registered with the GPC and compliant with its code of Professional Conduct
Experience managing practice medicines management workflow
Excellent communication skills, both written and spoken
IT literate, proficient with software systems like Emis Web, SystmOne, and Microsoft Office
Confident in managing conditions like Hypertension, Type 2 Diabetes, Asthma, and more
