About the role

Key responsibilities & impact

Successful candidate will have Home Health experience
Facilitates the Home Care and/or Hospice admission process
Attending hospital rounds
Facilitating orders
Assessing patient needs
Completing referral process and admitting patient as appropriate
Forwarding hospital documents to hospice
Communicating with hospice and hospital staff
Providing education to hospital staff and physicians
Able to make home visits to provide patient care coverage as needed

Requirements

What you’ll need

Graduate from an accredited licensed vocational nursing school
LVN-Licensed Vocational Nurse
BLS-Basic Life Support
Valid Drivers License Class C or B
5 years recent relevant experience
Understands the diagnostic criteria for GIP eligibility
Must have clinical knowledge of disease entities common to hospice/home health
Commitment to teamwork and customer service
Ability to solve problems with customers and management
Organizational skills
Must have telephone, interpersonal and written communication skills.
