About the role
Key responsibilities & impact- Conduct telephone outreach to designated member populations to ensure retention, service utilization, satisfaction, and compliance reporting
- Assist with care coordination for members with special healthcare needs designated by CCOs, OHA, members, caregivers, or representatives
- Contact and counsel members regarding emergency department usage for dental-related reasons
- Contact and counsel members regarding missing dental appointments
- Assist in reassigning dismissed members from their current primary care dental office and coordinate future care
- Escalate cases to a Care Coordinator, Advanced when necessary
- Coordinate additional dental-practice services after members are seen in a community setting
- Process Community Partner requests for assistance through Health Risk Screening
- Process medical-to-dental referrals to care
- Process pilot-project referrals to care
- Enter documentation accurately and timely into case management systems
- Improve regional health by considering members’ unique health, cultural, and social needs
- Perform other duties as needed or required
Requirements
What you’ll need- Must reside in Oregon
- High school diploma or equivalent
- 6+ months of experience working with Medicaid populations by phone or in person, or equivalent
- Computer proficiency in a variety of Windows applications
- Attend additional training as requested/deemed necessary
- Preferred: Certification as a Community Health Worker or other Traditional Health Worker designation
- Preferred: 2+ years of experience working in a call center environment
- Preferred: Experience in a dental/medical insurance setting
- Ability to communicate effectively
- Ability to perform overall light physical effort up to 25 lbs.
- Manual dexterity and sitting required
- Ability to travel or move about within and outside serviced facilities
