About the role

Key responsibilities & impact
  • Participates in patient treatment planning and continuum of care.
  • Conducts group and/or family sessions, under supervision, as scheduled and documents as required.
  • Co-facilitates assigned group or family sessions and assists with modifying problem behaviors.
  • Responsible for initial assessments, as well as follow up assessments for patients.
  • Prepares individual treatment plan for each assigned patient.
  • Ensures all documentation regarding patient care, treatment and incidents is completed timely and in a clear, concise manner.
  • Acts as a liaison between referral sources and patients.
  • Provides crisis intervention to patients, as needed.
  • Plans for aftercare for assigned patients.
  • Acts as a liaison with insurance carriers, as requested.
  • Provides case management duties for patients, ensuring individualized quality care.
  • Evaluates the need of the patient and determines if referrals to other programs or facilities is needed.
  • Provides patient and family education relative to substance abuse/psychiatric problems.
  • Conducts educational workshops both within the Hospital and in the community as required.
  • Serves as a liaison with self-help groups and EAP professionals.
  • Conducts ongoing assessments of chemical dependence services required by the Hospital treatment programs.
  • Assists in the training of new clinical staff and interns, providing direction and clinical guidance

Requirements

What you’ll need
  • MUST BE INDEPENDENT LICENSED (LCSW, LPC, LSATP, or LMFT) or under supervision with a MS DEGREE IN PSYCHOLOGY, SOCIAL WORK, or EDUCATION WITH CLINICAL FOCUS
  • Requires Bachelor's degree in social or health services field, with 4+ years exp in counseling field;
  • OR Bachelor's degree in other field with 6+ years in the counseling field;
  • OR Master's degree in social or health services field, with 2+ year professional work experience.
  • Degree must be from an accredited college or university.
  • Licensure or certification for counseling may be required by the state in which the facility operates.
  • CPR and de-escalation certification required (training available upon hire and offered by facility).
  • First aid may be required based on state or facility.
