About the role

Key responsibilities & impact

Undertake assessments, service commissioning and reassessment work.
Conduct assessments via video/telephone and/or face to face.
Collaborate with Social Workers, Occupational Therapists and Assistant Practitioners.
Build strong partnerships with service users and their families.
Maintain focus on strengthening social care across Norfolk.

Requirements

What you’ll need

Experience of working with vulnerable people.
Excellent communication, record-keeping and keyboard skills.
Familiarity with the Departments' eligibility criteria.
Sound knowledge of the Care Act (2014), Mental Capacity Act (2005) and Safeguarding.
A commitment to excellent customer service.
