Human Services Counselor III

About the role
Key responsibilities & impact- This is a professional position providing confidential case management/guidance to vulnerable adults
- Prepares and maintains detailed client assessments and service plans for each client
- Provides/arranges for all services identified in the service plan
- Visits clients regularly, documents observes and revisits client assessment and service plans
- Assesses the need for emergency intervention and/or on-going services
- Maintains liaison with community resource agencies
- Testifies in court hearings, attend fair hearings and legal proceedings
- Attends in-service and pre-service training as required
Requirements
What you’ll need- Associate’s degree or minimum of 60 semester hours and two years of professional experience in education, mental health, counseling, guidance, social work, health or rehabilitative programs
- Professional experience can substitute on a year-for-year basis for the required college education.
- Required to travel to clients' homes or to facilities to complete casework
- Must have a valid driving license, vehicle registration, and appropriate automobile insurance
