About the role
- Provide pediatric and family social work expertise for the Medical Foster Care (MFC) program reporting to the Registered Nursing Supervisor
- Complete pre-service, annual, quarterly, and as-needed psychosocial assessments of prospective and current MFC parents and MFC child/youth dyads, including home and community visits
- Complete documentation and maintain electronic records in accordance with program requirements
- Assess availability of MFC beds statewide and conduct bed searches when local beds unavailable
- Provide verbal and written communication, education, training, and technical assistance to MFC and community stakeholders
- Coordinate MFC staffing, admissions, plans of care, equipment, medications, and resources for MFC child/youth placements
- Provide ongoing medical social work support, retention efforts for MFC parents, and arrange temporary caregiver relief services as needed
- Accompany MFC providers and children/youth to appointments and participate in multidisciplinary monthly staffing and CMAT meetings
- Provide after-hours on-call program support as assigned
- Maintain, review, and monitor MFC related data and Electronic Health Record entries and participate in quality assurance and improvement activities
- Assist with Medicaid enrollment processes and coordination with MMA plans, and provide psychosocial or administrative documentation or testimony when requested by court order
Requirements
- Knowledge of medical terminology
- Knowledge of foster care system and child/family community resources
- Skill in performing psychosocial assessments
- Skill in detailed documentation
- Ability to work with families and children with special health care needs
- Ability to communicate effectively verbally and in writing
- Ability to plan, organize and coordinate work assignments
- Ability to collect and organize data
- Ability to access information from computer data systems and websites
- Ability to use a computer for word processing, spreadsheets, and database applications, and for attending virtual meetings and training
- Must hold a valid unrestricted Florida Driver License
- Must have computer skills, including use of standard office machinery and programs such as Outlook, PowerPoint, Excel and SharePoint
- Travel 25% - 30%, to include home visits, community visits, medical provider visits
- Successful completion of a security background check and/or drug screening
- Participation in direct deposit (required)
- Must provide Social Security Number for background check
- Male applicants born on or after October 1, 1962, must be registered with Selective Services or have exemption
- All Florida Department of Health positions require ability to learn and communicate effectively in English
- Preferred: Bachelor’s degree or higher in the social services field
- Preferred: 2 years of child welfare experience and medical setting experience
