💰 $35,000 - $50,820 per year

About the role

  • Provide social services, counseling guidance, and discharge planning for individuals with mental disabilities
  • Maintain monthly contact with case management and interact with Community Case Managers
  • Complete necessary social services progress notes and assessments
  • Attend legal hearings and represent the unit and social services department
  • Facilitate counseling and therapeutic sessions based on recovery plans
  • Inform residents about their rights and coordinate discharge planning.

Requirements

  • A Bachelor’s degree in Social Work or a related degree from an accredited college or university
  • Three years of professional experience in developmental disabilities, special education, mental health, counseling, guidance, social work health or rehabilitative programs
  • A master's degree in Social work or a related field can substitute for one year of the required experience.
