About the role
- Provide social services, counseling guidance, and discharge planning for individuals with mental disabilities
- Maintain monthly contact with case management and interact with Community Case Managers
- Complete necessary social services progress notes and assessments
- Attend legal hearings and represent the unit and social services department
- Facilitate counseling and therapeutic sessions based on recovery plans
- Inform residents about their rights and coordinate discharge planning.
Requirements
- A Bachelor’s degree in Social Work or a related degree from an accredited college or university
- Three years of professional experience in developmental disabilities, special education, mental health, counseling, guidance, social work health or rehabilitative programs
- A master's degree in Social work or a related field can substitute for one year of the required experience.
