About the role

  • This nursing position serves as a home visitor to Healthy Start clients
  • The Nurse Home Visitor is responsible for providing face-to-face visiting services to Healthy Start clients in the home, office, or clinical setting
  • Deliver education, care coordination, screening and interventions/referrals
  • Follow-up in accessing prenatal care and other needed services
  • Provide trauma informed care to pregnant women, children birth to aged 3, and their families
  • Case management of a large Healthy Start maternal and infant caseload
  • Ensure continuity of care for clients
  • Collaborate with other agencies while maintaining client confidentiality

Requirements

  • Minimum – Florida driver's license, or the ability to obtain one
  • Active Florida Registered Nursing license in accordance with Florida Statute 464 or a multistate license from another Compact State
  • Preferred – Home visitation experience
  • Maternal/child health experience
  • Case management experience
  • Bachelor’s degree in nursing, social work, guidance and counseling, or a related field of study
