About the role
- This nursing position serves as a home visitor to Healthy Start clients
- The Nurse Home Visitor is responsible for providing face-to-face visiting services to Healthy Start clients in the home, office, or clinical setting
- Deliver education, care coordination, screening and interventions/referrals
- Follow-up in accessing prenatal care and other needed services
- Provide trauma informed care to pregnant women, children birth to aged 3, and their families
- Case management of a large Healthy Start maternal and infant caseload
- Ensure continuity of care for clients
- Collaborate with other agencies while maintaining client confidentiality
Requirements
- Minimum – Florida driver's license, or the ability to obtain one
- Active Florida Registered Nursing license in accordance with Florida Statute 464 or a multistate license from another Compact State
- Preferred – Home visitation experience
- Maternal/child health experience
- Case management experience
- Bachelor’s degree in nursing, social work, guidance and counseling, or a related field of study
