About the role
- This position is a grant funded position in the Healthy Start Program.
- Provide care coordination to the Healthy Start Program with the responsibility of decreasing structural, financial and personal barriers to achieve needed health services for eligible high-risk prenatal and infant clients.
- Make home visits to facilitate risk assessment, follow-up on missed appointments, link clients with services and provide opportunity for client self-sufficiency.
- Works closely with department heads of other agencies in the community and with supervisors within the Marion County Health Department to coordinate the activities of this program.
- Works under the direct supervision of the Senior Human Services Counselor Administrator – SES.
- A valid driver’s license and an automobile are required for this position; this position requires travel.
- Must be willing to represent Healthy Start during community health fairs and/or community events which may take place on occasional weekends.
- Makes home visits to evaluate eligible high risk prenatal and infant client needs.
- Develop and complete a Family Support Plan if applicable, during these visits identifying structural, financial and personal barriers to getting needed health services.
- Counsel eligible, high-risk prenatal clients in the home setting on the importance of prenatal care, proper nutrition and risk factors while counseling parents of high-risk infants on the importance of regular health screening, growth and development, nutrition, immunization schedules and safety.
- Document services provided and maintain records and reports of services rendered within 72 hours.
- Works closely with Marion County Health Department clinics, private health care providers and other community agencies in assuring clients medical/social needs are met.
- Implements group educational services to high-risk prenatal and infant clients within the Healthy Start program guidelines.
- Prepares and submits reports to meet local, district and state requirements.
- Maintains professional knowledge and skills by attending staff meetings, workshops, seminars and other professional training programs.
- Performs other duties as assigned.
Requirements
- FL Drivers License
- Associate Degree and / or 3 years experience providing medical or social services to women or children, pursuant to Chapter 6 of Healthy Start Program Standards and Guidelines - 2019
- Willingness to physically come into the office and perform field work
- Experience in maternal and child health (preferred)
- Experience in case management (preferred)
- Experience working with community partners (preferred)
